Do a quick Google search for “Pastor Evaluation Forms” and what comes back may help us understand in part why pastoral leadership in America is in a crisis. If the forms available are any indication, pastors are not being measured by any obvious biblical understanding of the office. (That said, I trust many churches have solid evaluation forms and evaluation practices not represented by a Google search. At least let’s hope so.)
So what do you do when you can’t find what you want? Build your own.
At Oak Park, our evaluations are coming up next month. In an effort to make these meetings as beneficial for all as possible, I built my own evaluation form. I offer it here for the benefit of any pastor who needs one for the sake of his ministry and the health of the church he serves.
